Seizures and Epilepsy

Seizures and epilepsy are two conditions that, although they can occur independently, may also be closely related.

What Is a Seizure

A seizure is a sudden, abnormal electrical activity in the brain that can cause a range of symptoms, from uncontrolled movements to loss of consciousness.

There are multiple conditions that can cause seizures, such as high fever, especially in children, known as febrile seizures, brain infections such as encephalitis or meningitis, head injuries, brain tumors, withdrawal from addictive substances like drugs or alcohol, metabolic imbalances, genetic or congenital disorders, and epilepsy.

There are different types of seizures, classified as focal or partial seizures, which affect a specific part of the brain, and generalized seizures, which affect both hemispheres of the brain. These may also be referred to as seizure episodes.

What Is Epilepsy

Epilepsy is a chronic neurological disorder characterized by a predisposition to recurrent and unprovoked seizures. Although the cause of epilepsy cannot always be identified, in some cases it can be linked to specific factors.

Brain damage due to prenatal or perinatal injuries, congenital abnormalities, genetic disorders, neurological diseases, prior brain infections, and head trauma have been associated with epilepsy.

The Difference Between a Seizure and Epilepsy

The difference between a seizure and epilepsy lies specifically in the origin and nature of the episodes.

A seizure is an isolated episode of abnormal electrical activity in the brain that can cause involuntary and uncontrolled movements, behavioral changes, unusual sensations, or even loss of consciousness. It has multiple possible causes and may occur only once or several times.

On the other hand, epilepsy is a chronic neurological disorder characterized by recurrent and unprovoked seizures.

This means that a person who experiences seizures does not necessarily have epilepsy. However, a person diagnosed with epilepsy will experience seizures to a greater or lesser extent if the condition is not treated. For this reason, it is common for people to believe that epilepsy and seizures are the same.

Types of Epilepsy

To classify epilepsy, it is necessary to identify the point or points of origin of the seizures in the brain. Signs and symptoms of epilepsy may include:

• Anxiety.

• Changes in the senses (vision and smell).

• Temporary confusion or absence episodes.

• Muscle weakness.

• Spasmodic movements in the arms or legs.

• Loss of consciousness.

• Muscle stiffness.

• Feelings of fear.

• A sensation of déjà vu.

Generally, a person living with epileptic seizures will experience the same symptoms or signs during each episode, which helps identify the area of origin.

Diagnosis of Epilepsy

For an accurate diagnosis of epilepsy, the doctor needs as much information as possible about what occurs during the seizures, including sensations, feelings, and physical reactions.

Additional tests may include:

• Blood tests: help determine whether there is an infection or condition that may be triggering the seizures.

• Intracranial electrodes: used for diagnosis or treatment in certain cases.

• High-density electroencephalogram (EEG): allows for more precise identification of the brain areas affected during seizures.

• Electroencephalogram (EEG): the most common test for diagnosing epilepsy, as it records the brain’s electrical activity.

• Neurological examination: evaluates behavior, motor skills, mental function, and other factors.

• Functional magnetic resonance imaging (fMRI): measures changes in blood flow in specific areas of the brain.

• Magnetic resonance imaging (MRI): creates detailed images of the brain to detect abnormalities or lesions.

• Computed tomography (CT scan): uses X-rays to obtain detailed images of the brain, revealing abnormalities such as tumors, cysts, or bleeding.

• PET scan: visualizes the brain’s metabolic activity and identifies abnormalities if present.

Once epilepsy is diagnosed, the doctor can classify it as focal or generalized seizures.

Each classification has subdivisions depending on the specific characteristics of each patient.

Treatment generally begins with anticonvulsant medications. Based on the patient’s response, adjustments may be made, or alternative treatment approaches may be considered if necessary.
